Your PUK code can be given to you by your newtwork if you contact them and pass security for your account. Normally it happens when you put a wrong PIN code into your phone 3 times. It's a SIM card lock. If you put the PUK into your phone that network provider gives you, the SIM would be unlocked again.

PUK stands for Personal Unblocking Code. This acts as a security feature on your mobile. To protect you in the event of theft, the SIM card is immediately locked if someone keys in three different incorrect PIN codes in succession.
